SPPS Athletics & Activities
The Athletic program at Sts. Peter & Paul is an extension of the learning experience for our students. We believe that participation in athletics provides students the opportunity to continue to develop physically, intellectually, emotionally, and socially in a Catholic environment beyond the school day.
Before participating in any sporting activities at Sts. Peter & Paul each student is required to have all of the following forms filled out completely. Athletic Forms can be obtained by clicking on the hyperlinks below.
First, read the Athletic Handbook for the current school year, fill out the form to acknowledge that you have read the handbook, and agree to its rules and regulations. If you have any questions or don’t see a form you need, contact us at beddy@sppslex.org.
Fill out the Handbook Acknowledgement Form and return it to the office or to Mrs. Eddy. One form can be filled out per family, however, all the student/athletes must sign the form.
Physicals
Physicals can be scanned and emailed to Beth Eddy (beddy@sppslex.org), turned in to the office, or faxed to school (859-254-9050).
Grades 5-8-Fill out the pre-participation form print and take it and the Physical Form to the doctor and then turn it into us. Be sure to turn in the Physical Form signed and dated by the doctor (6 pages total).

Grades K-4 Fill out alternate Physical and Participation Form and turn it into us. Be sure to turn in the Physical Form signed and dated by the doctor.

Girls Volleyball (grades 5-8)
Is your student athlete interested in becoming part of an amazing group of volleyballers?
We will have our first Open Gyms/Parent Information sessions from 5:30-7:00 pm on Tuesday, Wednesday 7/28, and Thursday 7/29 in the SPPS gym. The first 15 minutes each day will involve Parent/Athlete Orientation and paperwork collection. Parents/Guardians will then leave
**It is very important parents/rides are in the parking lot at the designated time for pick-up at the end of practices.
ALL players must have a completed KHSAA Pre-Participation Physical Evaluation, Physical Exam Form, and Student/Parent Release Form (6 pages) prior to open gym. The Student Athlete Handbook also has a Sportsmanship Pledge/Acknowledgement page which will need to be signed by both the athlete and a parent/guardian after reading through the handbook.

Basketball (grades 5-8)
If your student is interested in playing basketball later this year please fill out the interest form. This information will ensure you receive future communication via email about our upcoming 2021-22 Basketball season.

For ALL Athletics/Activities the following items along with the activity-specific permission slip MUST be turned in prior to participating in the activity. These can be turned in via email to Beth Eddy beddy@sppslex.org and to the office ℅ Beth Eddy. All forms must be turned in and verified prior to participation. Please be sure you allow for ample time when scheduling a physical.
